Colors shown by computer are approximate and will vary due to monitor differences. Customers who want dark kraft, but choose a graphic with dark colors, may want to consider the light kraft so the graphics show up.






On the back side of the form, there are horizontal lines across the page, so you can write name/address/and do quantities and its separated from the next customer. The lines just don't show up here on the internet.










Fund Raising forms are a great way for you to sell items and help a good cause at the same time. The forms can be used for candles and any other types of items you sell. We will work with you for choosing colors that relate to your products or organization. Font style and border design may vary.

Fund Raising Form Set Up Fee
$8.00 for set up of a fund raising form with up to 8 candle scents (as shown). $10.00 for set up with 11-15 items. $12.00 for set up with 16-20 items.
Fee includes setting up the form with text, pictures that you provide, and graphics if there is room. Pictures must be emailed to us a .jpg's.
Forms are printed on cardstock (white, ivory, cream, light kraft, dark kraft, gray, etc). We will email you for paper color selection.
